School personnel may administer medication to pupils only when
the school has received
an “Authorization for Administration of Medication by School
Personnel Form” If your child is receiving medication
during school hours, please come to the school office for a form.
No medication, including over-the-counter medications,
shall be allowed on a school site or administered to any
pupil without a document from a licensed physician and the
parent/guardian of the pupil.
